Launching Flexpa's Endpoint Directory

Introducing Flexpa's Endpoint Directory - a comprehensive, real-time index of Payer Patient Access APIs with detailed implementation status, authorization flows, and FHIR capabilities.

Today, we're excited to announce the launch of Flexpa's Endpoint Directory. Our directory is the largest of its kind - CMS-9115 Payer Patient Access APIs - and we're releasing it to the public for free.

The directory provides detailed, real-time information about each payer's connection status, authorization requirements, and FHIR API capabilities.

Flexpa Endpoint Directory

Why an Endpoint Directory Matters

The healthcare interoperability landscape has evolved rapidly since CMS-9115 mandated health insurers to create patient access APIs. While this regulation created unprecedented opportunities for healthcare innovation, it also introduced a significant challenge: how do developers, let alone individual patients, discover and connect to hundreds of different payer endpoints, each with their own implementation quirks?

This challenge manifests in several ways:

  1. Discoverability: There's no centralized registry of which payers have implemented Patient Access APIs
  2. Implementation Details: Even when endpoints are known, their specific requirements around authorization flows, token handling, and FHIR resource support vary significantly
  3. Real-time Status: Endpoints can experience outages or changes that affect their availability
  4. Technical Requirements: Different payers have unique requirements for headers, parameters, and authentication flows

The Flexpa Endpoint Directory Solution

Our directory addresses these challenges by providing:

  • Comprehensive Coverage: Details on over 488 endpoints across 320+ payers
  • Real-time Status Monitoring: Up-to-date tracking of endpoint availability and performance
  • Implementation Details: Specific information about authorization flows, token types, and refresh capabilities
  • FHIR Resource Support: Clear documentation of available FHIR resources and operations
  • Success Rate Metrics: Real-world performance data based on actual patient interactions

For each endpoint, we track and display:

  • Connection status (Live, User Validated, Broken, etc.)
  • Token details (refreshable vs. non-refreshable, maximum authorization periods)
  • Authorization success rates
  • Available FHIR resources
  • Lines of business supported
  • Last verified date
  • Developer portal links when available

Why Flexpa?

As the operator of the largest network of payer FHIR endpoints in the U.S., Flexpa has unique visibility into the real-world implementation status of Patient Access APIs. Our directory is built on:

  • Active Monitoring: We continuously test and validate endpoints through our production traffic
  • Deep Implementation Knowledge: Our experience integrating with hundreds of payers informs our understanding of each endpoint's nuances
  • Real User Data: Our success metrics are based on actual patient authorization attempts and data retrievals
  • Direct Payer Relationships: We work closely with payers and vendors to maintain accurate implementation details

Using the Directory

The directory is available here. You can:

  • Search for specific payers
  • Filter by implementation status
  • View detailed technical specifications
  • Track authorization success rates
  • Monitor endpoint reliability

For developers, the directory provides the technical details needed to implement successful integrations. For payers, it offers visibility into how their implementation compares to industry standards and peers.

Looking Ahead

The Endpoint Directory is a living resource that will continue to evolve. As we approach new interoperability milestones like CMS-0057-F, we'll expand the directory to include additional metrics and capabilities relevant to Payer-to-Payer and Provider Access APIs.

We invite payers and vendors to partner with us in maintaining the accuracy of their listings. If you'd like to update your endpoint information or discuss testing partnerships, please reach out to us at interop@flexpa.com.
